1960 November: Delivered new to No. 435 (T) Squadron, RCAF Station Namao {Edmonton}, Alberta, Canada.[RWR Walker]
1962 August: Took part in Operation Boxtop VII, resupply of CFS Alert, North West Territories{Nunavet}, Canada, from Thule, Greenland.[RWR Walker]
1964 November: Seen at Dusseldorf - International (Rhein-Ruhr / Lohausen) (DUS /EDDL) Germany. Assigned to No. 435 Squadron, homebase Edmonton, Alberta, Canada.[Günter Grondstein]
1965 or 1966: Used by No. 408 Squadron at Rivers Camp, Manitoba, Canada.[RWR Walker]
1966 April 15: Pancaked into field near North Battleford, Saskatchewan, Canada, after explosive decompression.[Joseph F. Baugher] With No. 408 Squadron at time of accident.[RWR Walker]
1966 April 15: When they tried to put the flaps down for landing they lost the flight controls for lack of utility hydraulic pressure. The crew stopped the flap travel and the flight controls came back. There was some pretty serious stick handling on that landing! You can see the flaps are just not quite totally up.
It was the one engine that wasn’t shut down. The other three engines were dead from getting hit by the door. The Booster hydraulic system, the second hydraulic source for the flight controls, had emptied itself when the #3 engine got clobbered by the door and had no pressure and they found out later that the #1 engine, slowly windmilling with three blades in positive pitch and one in negative, was the only thing giving them very minimal Utility hydraulic pressure. That’s why the flight controls seized up when they were trying to lower the flaps. What essentially happened was the cargo door came off in cruise and when it did it went straight into #2 prop. #2 prop threw it into #1, #1 threw it back into #2, and #2 then threw the door over top of the fuselage into #3. #3 spit the door out without getting #4 but when the door went over top of the fuselage it severed all the engine controls for #4, the one engine still running. It dropped into a "cruise descent" type of power setting and they could neither increase or decrease power, but at least it gave them a generator which (assuming the power runs were still intact and they do have some redundancy for something like this) they would have had power to the Essential and Main AC and DC buses. That they got the thing down and walked away from it is nothing short of a miracle and speaks volumes for the quality of the crew and of their training.[Dan Jones]
