1970 August 14: Renumbered from RCN 4038 to CAF 12438.[Canadian Military Aircraft 1968-1998, Griffin, Smith and Castle]
1975/1976: Converted to CH-124A as a result of SKIP {(Sea King Improvement Program)} upgrade.[RWR Walker]
1983: With 423 Squadron, while onboard HMCS Skeena.[RWR Walker]
1991: Converted to Operation Friction configuration, no change in designation.[RWR Walker]
1993: Operated in support of UNUnited Nations mission in Somolia, with nose FLIR {(Forward Looking Infra-Red)} mount.[RWR Walker]
1995: At CFBCanadian Forces Base Shearwater, Nova Scotia, Canada.[RWR Walker]
2006 February 2: Crashed in North Sea, 18:34 UTC, in open water, at the Kattegat, NW of the island of Anholt, Denmark, while operating from HMCS Athabaska in NATO exercise. Ship was assigned to Standing NATO Maritime Group 1. Tail rotor struck water during flare while attempting night landing on board. Had been waved off previously, apprently due to problems on ship deck. Sank in 16 metres of water, all crew recovered.[RWR Walker]
