9 July 1952 - Struck off, after Category A crash near Picton, Ontario in June of 1952. Mustang 9555 1952-06-10. From the RCAF accident synopsis: "The pilot was climbing to 31,000 feet. An observer noted a sharp kink appear in the vapour trail. The Aircraft apparently spiralled down to a height of approximately 2,000 feet above the surface of the lake before the pilot attempted to recover. When the pilot tried to recover, the port wing failed and the Aircraft crashed, killing the pilot on impact. The investigation found that the oxygen blinker system and line had not been removed as per Special Inspection E0-20-115CB-5/1. The investigation also revealed the pilot was un-aware that the oxygen blinker system was not operative nor had the pilot been briefed on the substitute procedures for checking oxygen flow when the blinker system is isolated. The most probable cause of the accident was anoxia."
